In some cases, that's because the writers were unaware that they were doing so. After all, the show has been going almost fifty years. Not only is that a long time, but in at least the early days I don't think that facts were meticulously recorded in a "bible" the way that shows do nowadays, In other cases, the writers realise that they are doing it, and come up with some more or less convincing explanation.<br /><br />At some point in the next decade or two, there will be a major one, as a long time ago it was stated that the Doctor could regenerate at most thirteen times (IIRC).
